Monoferric vs Venofer: Which IV Iron is Right for You?

The two main IV iron products in Canada

In Ontario, two IV iron products are routinely prescribed: Monoferric (ferric derisomaltose) — Manufactured by Pharmacosmos. A newer-generation iron formulation introduced to Canada in 2018. Venofer (iron sucrose) — Manufactured by Vifor. The longest-established iron formulation in Canadian practice, on the market since the early 2000s. Both are safe and effective. The differences come down to dosing, number of visits, and cost — and the right choice depends on your situation.

Number of visits required

Monoferric: Often a single infusion is enough to fully replenish iron stores. Up to 1500 mg can be delivered in one 30-minute session for most adults. Venofer: Maximum dose per session is 200 mg, so most patients need 4–6 separate visits spaced 1–2 weeks apart to achieve full repletion. For people with busy schedules or who live far from the clinic, Monoferric's single-visit convenience is a major advantage.

Side effect profile

Both products are well-tolerated. Direct head-to-head studies show: Monoferric: Slightly lower rate of infusion-related reactions. Less skin staining risk. Approved for full dose in 30 minutes. Venofer: Excellent long-term safety data going back two decades. Slightly higher rate of mild side effects (transient back/joint pain, headache) reported in some studies, but still rare. For patients with significant allergy history or anxiety about medications, Monoferric's lower reaction rate makes it the slightly safer choice.

Cost comparison

Monoferric: Roughly $650–$800 per dose. Higher upfront cost, but fewer visits. Venofer: Roughly $50–$80 per session. Multiplied by 4–6 sessions = $200–$480 total medication cost. Cheaper upfront, but more visits required. Both are typically covered by extended health benefits. Government programs (Trillium, ODB) cover Venofer more readily than Monoferric in some cases.

Which one will we recommend?

At MED1, our physician makes the recommendation based on: Monoferric is usually preferred when: • You want a single-visit treatment • You have a busy schedule or live far from the clinic • Your insurance covers it • You have a history of poor tolerance to other IV iron products • You need rapid repletion before pregnancy, surgery, or training Venofer is preferred when: • Cost is a major concern and Monoferric isn't covered • You're on a government drug program that covers Venofer • Your physician prefers gradual repletion (e.g., very low body weight, specific medical conditions) • You've had a prior reaction to ferric derisomaltose Both options are excellent. The wrong choice doesn't really exist — only the choice that best fits your circumstances.
